Analysis of key technical points

1. Three-dimensional specification for specimen extraction

Chapter 4 of the standard clearly requires: Injury specimens must carry adjacent normal tissues, including:

  • Current spot: When the diameter is greater than 4cm, strips of tissue must be taken across the maximum diameter
  • Neck cord groove: Longitudinal incision perpendicular to the cord groove
  • Bone marrow extraction: For hematopoietic system diseases, more than 3 sites must be extracted (ilium/sternum/femur)

2. Scientific ratio of fixative

Chapter 5 of the standard specifies two core fixatives:

  1. Conventional 10% formalin: formaldehyde: water = 1:9 (prepared and used immediately)
  2. Neutral buffered formalin: add phosphate buffer (pH7.0-7.4)

Typical case: Acidic formalin fixation of bleeding foci is prone to produce brown-black precipitates, and the neutral formula can effectively avoid this phenomenon.


Special requirements for organ processing

Cardiac conduction system sampling

Standard 8.1.3 stipulates:

  • Sinoatrial node: 3-4cm tissue longitudinally cut at the junction of the superior vena cava and right atrium
  • Atrioventricular bundle: The vertical section on the left side of the coronary sinus includes the atrial septum and ventricular septum

Standardized brain cutting method

The 12-cut 13-piece coronal cutting method is adopted:

  • 4th cut: sampling from the middle frontal gyrus/cingulate gyrus
  • 7th cut: sampling from the mammillary body
  • 8th cut: double sampling of the hippocampus/thalamus

Implementation suggestions

  1. Sample container management: Special containers with a diameter of ≥30cm and a height of ≥32cm. Mixing of samples from multiple cases is prohibited
  2. Infectious disease prevention and control: HIV and other positive samples must be marked with a red label and stored separately
  3. Sample packaging: Long-distance transportation requires 3-4 layers of formalin gauze + sealed plastic bag + hard outer box
