1. Dynamic adjustment mechanism of anticoagulant
When the hematocrit>>55%, the dosage of sodium citrate needs to be adjusted according to the formula:
C=(1.85×10⁻³)×(100-Hct)×V
Where C is the volume of anticoagulant (ml), V is the blood collection volume (ml), and Hct is the hematocrit (%).
2. Specimen transportation temperature control
- Conventional transportation: 18-24℃ (refrigeration is prohibited)
- When delayed testing: PT specimens must be separated into plasma within 1 hour and transported on -70℃ dry ice
- Platelet function test: Maintain 18-24℃ throughout the process
Implementation recommendations
Case: Anticoagulation treatment of patients with high Hct
A patient with polycythemia vera has Hct=60%, and needs to test PT/APTT:
1. Calculate the anticoagulant adjustment amount (when 2.7ml of blood is collected):
C=(1.85×10⁻³)×(100-60)×2.7=0.2ml
2. Use dedicated low-volume blood collection tubes or manually adjust the amount of anticoagulant added
Quality improvement measures
- Establish a centrifuge verification system (verify platelet removal effect every 6 months)
- Add a specimen time monitoring module to the information system
- Provide special training for pediatric blood collection personnel on the use of butterfly needles
