本标准规定了包虫病疫情控制与传播控制的要求和考核方法。
本标准适用于我国包虫病流行区不同防治阶段目标的考核。

Introduction

Analysis of the core framework of the standard

This standard constructs a two-level prevention and control system: epidemic control (basic requirements) and transmission control (advanced goals). The technical indicators are designed based on the epidemic characteristics of echinococcosis in my country, focusing on monitoring the results of ultrasound diagnostic instruments and animal host infection data.

Assessment dimensions Epidemic control threshold Transmission control threshold Detection method
Residential prevalence <1% - WS 257 diagnostic criteria
Children's prevalence - <1‰ Abdominal ultrasound examination
Canine infection rate <5% <1% Autopsy/arecoline catharsis

Operational specifications for key indicators

1. Technical requirements for population screening

When using portable ultrasound equipment for examination, please note:

  • The examination rate for residents must be >80%, and for children >95%
  • Liver scans use intercostal oblique sections, and the diameter of the cyst must be recorded to the nearest mm
  • Serum samples must be collected from suspected cases for immunological review

2. Key points for animal host monitoring

The arecoline hydrobromide catharsis method is recommended for canine testing:

  1. Administer the drug (4 mg/kg) after fasting for 12 hours
  2. Collect all feces within 2 hours
  3. Use the sedimentation method to examine the protoscolex under a microscope

Recommendations for the implementation of the standard

Application Cases in Qinghai Province

The "three deworming and two inspections" model was adopted in Yushu Prefecture:

  • Quarterly dog deworming (praziquantel 5mg/kg)
  • Mandatory organ inspection in slaughterhouses
  • Annual screening of school-age children
After three years of implementation, the dog infection rate dropped from 7.2% to 0.8%.

File management specifications

An electronic database needs to be established, including:

  • Dog registration and deworming records
  • Case follow-up data (updated every 6 months)
  • Original records of intermediate host detection
