Introduction
Interpretation of the core content of the standard
This standard is the first to formulate specific weight gain recommendations for Chinese women with gestational diabetes (GDM), and divides the population into four categories based on pre-pregnancy BMI: underweight (BMI<18.5), normal weight (18.5≤BMI≤24), overweight (24<BMI≤28), and obesity (BMI≥28).
Weight gain management by stage
| Pregnancy stage | Measurement indicators | Clinical significance |
|---|---|---|
| Early pregnancy | 0-2.0kg (full BMI classification) | Control overcompensation during morning sickness |
| Before and middle stages of GDM diagnosis | 0.15-0.56kg per week | Predict the risk of insulin resistance |
| Middle and late stages after GDM diagnosis | 0.12-0.56kg per week | Critical period for blood sugar control |
Technology Evolution Analysis
Compared with the WS/T 801 standard for ordinary pregnant women, the innovations of this standard include:
1. For the first time, it distinguishes the weight gain rate before and after the diagnosis of GDM
2. Sets a stricter weight gain upper limit for obese pregnant women (0.18kg/week)
3. Clearly excludes special groups with a height of <140cm or a pre-pregnancy weight of >125kg
Implementation Recommendations
Key points of clinical operation:
• It is recommended to use a calibrated electronic weight scale (accuracy 0.1kg)
• Measurement should be performed in the morning on an empty stomach and with an empty bladder
• Obese pregnant women need to adjust their dietary structure in conjunction with blood glucose monitoring
Typical case:
A certain BMI before pregnancy 26.5 (overweight) pregnant women with GDM: - Weight gain should be controlled to 0.22-0.37kg per week before and during the middle period of diagnosis. - Weight gain should be reduced to 0.19-0.32kg/week during the middle and late period of diagnosis.
