Laboratory Testing Technical Specifications
Three-tier Detection System
- Screening test: ELISA/chemiluminescence method, results expressed as reactive/non-reactive
- Supplementary tests:
- Western blot test (WB)
- Nucleic acid test (qualitative/quantitative)
- Confirmation basis: Nucleic acid>5000 CPs/mL or two positive results
Key points for the implementation of clinical staging
Adult staging standards
| Stages | CD4 Count | Key Indicators |
|---|---|---|
| Stage I (Early Stage) | >500 | Acute Infectious Syndrome/PGL |
| Stage II (Intermediate Stage) | 200-500 | Recurrent Bacterial Infections/Oral Leukoplakia |
| Stage III (AIDS) | <200 | Opportunistic Infections/Malignant Tumors |
Special Considerations for Children
- Under 18 Months: Two Positive Nucleic Acid Tests Required (≥4 Weeks Apart)
- Immune assessment: CD4 percentage is used (<25% at 12 months of age is considered severe deficiency)
Recommendations for implementation of the standard
- Window period management: Nucleic acid testing can be performed 1 week after exposure, and antibody testing 3 weeks later
- Child diagnosis: Children whose mothers are HIV-positive should complete the first nucleic acid screening within 48 hours of birth
- Result interpretation: Reactive samples must be confirmed by additional tests to avoid false positives
